Affliction..

 

    Tells the story of Wade Whitehouse (Nick Nolte), a small-town policeman whose investigation of an apparent hunting accident is affected by his relationship with a violent, alcoholic father (who he's afraid he's turning into), his increasing obsession with his past, his stressed relationship with his ex-wife and daughter and the death of his mother. All of these issues cause a personal crisis for Wade as the investigation proceeds. During the course of the film he's having trouble with a toothache as well and it all comes to a head in this scene when he decides to take matters into his own hands. One of my favorite scenes from any movie because while his personal life is in shambles it's really the tooth that's driving him nuts..
